The 67-year-old Die Hard star\u2019s family revealed yesterday that he had frontotemporal dementia (FTD), a progression from his diagnosis of aphasia last year.<\/p>\n
This rare type of dementia is especially devastating because it strikes earlier than other forms \u2014 like Alzheimer\u2019s<\/a> \u2014 when patients are at the pinnacle of their careers and may have still have children at home.<\/p>\n
Doctors say there is no medication to slow the pace of the disease and that three to five years after diagnosis patients tend to end up in a care home. Some colleagues noticed shifts in his personality years ago, saying they feared he did not know where he was. He has also struggled to remember his lines<\/a>, leaving him needing an ear-piece on set.<\/p>\n
Not as much research has been carried out into FTD because it is rare, making up one in 20 dementia cases. But medics say that situation is changing and that there is growing awareness of the disease because of stars like Willis.<\/p>\n

What is it like having FTD?<\/span><\/p>\n Doctors said that patients with FTD find it \u2018confusing\u2019 because it strikes earlier than other forms of the disease.<\/p>\n It is a rare type of dementia \u2014 accounting for around one in 20 cases. About 50,000 to 60,000 Americans are thought to have the disease.<\/p>\n But it is also one of the most common types of the disease among people aged 40 to 60 years old, well before when you would expect similar diseases like Alzheimer\u2019s to appear \u2014 which normally strikes after someone turns 65.<\/p>\n Dr Susan Dickinson, the chief executive doctor of the Association of Frontotemporal Degeneration, told DailyMail.com: \u2018Think about this, you are nearing the height of your career, you may have kids at home, you are co-parenting with your partner, your spouse, so on.<\/p>\n \u2018It is not a time of life when most doctors think dementia.\u2019<\/p>\n \u2018And so on the way, most people get a series of misdiagnoses. The most common are depression, bipolar disorder and Alzheimer\u2019s.<\/p>\n \u2018And in the meantime, the damage has been done to one\u2019s relationships. With family finances for example, as I said because of errors of judgment, patients may have spent the kids\u2019 college fund on a sports car or two.\u00a0<\/p>\n \u2018A lot of people lose their jobs for making mistakes before the disease is diagnosed.<\/p>\n \u2018So you lose healthcare, maybe healthcare for the whole family.<\/p>\n \u2018It is really, really devastating, and at that younger age it has a very different impact on a family.\u2019<\/p>\n She said that early on many patients face confusion over their actions and are unsure what is causing the changes in behavior.<\/p>\n It can take about three years to get the disease diagnosed in many cases, during which time patients receive inappropriate treatment.\u00a0<\/p>\n How does the disease progress?\u00a0<\/span><\/p>\n Doctors warned that patients with the disease gradually deteriorate over time.<\/p>\n The speed of this varies between patients.<\/p>\n In the early stages, an FTD sufferer may be able to function normally but struggle to recall words.<\/p>\n In later stages, however, patients need round-the-clock care and may experience shifts in their personality as well as language. It may also affect other abilities, such as movement.<\/p>\n Dr Keith Vossel, a neurologist at the University of California, Los Angeles, told DailyMail.com: \u2018I would divide FTD into roughly three stages.<\/p>\n \u2018Initially, people have difficulty with language but they are functioning quite well. They will still be independently able to drive and do usual activities.<\/p>\n \u2018The trouble with language involves a loss of vocabulary and having trouble identifying objects and naming them, like saying \u201cthe thing\u201d instead of the coffee maker.<\/p>\n \u2018But in the moderate stage, patients begin to need some assistance with daily activities. They may well need a communication device or some kind of aid for conveying their ideas.<\/p>\n \u2018Language can still be perceived in tone and melodic nature, but the richness of the vocabulary gets diminished.\u00a0<\/p>\n \u2018In the more severe stages with FTD, patients often develop apathy or some other behavioral disturbances.<\/p>\n \u2018This includes personality changes such as obsessive-compulsive behaviors \u2014 like buying the same book repeatedly or going to a record store every day \u2014 and loss of empathy, so people may not seem as concerned about others as normal.\u2019<\/p>\n The pace of the disease progression varies between patients and there is no way to slow its speed. Doctors are not sure what determines the disease progression.<\/p>\n There are several types of FTD, but doctors are confident that he likely has the primary progressive aphasia (PPA) type.<\/p>\n Dr Vossel said: \u2018People with PPA can live 12 years, but it may take longer to get diagnosed.<\/p>\n \u2018There are some other potential complications that patients can have including motor symptom issues \u2014 so trouble with movements.\u2019<\/p>\n It is not clear how far along in the disease progression Willis is, but those in the acting industry say he was struggling for years.<\/p>\n Pictured: Bruce Willis on set of American Seige with an earpiece\u00a0<\/p>\n Those working with him suggested last year that they felt he was no longer fully aware of his surroundings and was struggling to remember his dialogue, reports the Los Angeles Times<\/a>.<\/p>\n He was also spotted in one of his later films \u2014 American Seige \u2014 w<\/a>earing <\/a>an earpiece on set so that his lines could be fed to him.<\/p>\n Doctors say how fast the disease progresses differs for each patient. They are not sure what determines the speed of the disease progression.\u00a0<\/p>\n Why is there no treatment to slow the progression of the disease?<\/span><\/p>\n A treatment is not yet available to slow down the progression of FTD, doctors have said.<\/p>\n They suggested that this was because it was a rare form of dementia, with research funding instead going to other types.<\/p>\n But sounding a positive note, Dr Vossel said the situation was changing. The speech therapy will help with this through reading and comprehension.\u2019<\/p>\n What tests may Bruce Willis have needed?<\/span><\/p>\n Doctors said that in order to diagnose the FTD he will have had testing of biomarkers in his blood and also PET imaging scanning in his brain.<\/p>\n Blood biomarkers tested for include tau proteins and progranulin, another protein involved in regulating cell growth and inflammation.<\/p>\n PET scans will have searched for any proteins building up in the brain, which would indicate the disease, and where they were building up.<\/p>\n In FTD, tau proteins build up in the frontal and temporal lobes, at the front and sides of the brain \u2014 areas involved in language and personality.<\/p>\n Scientists would also have tested his genetics to look for any that may predispose him to FTD.\u00a0<\/p>\n What caused his FTD?<\/span><\/p>\n Doctors said it was not clear what had led to the legendary actor developing FTD.\u00a0<\/p>\n Medics say the disease is caused by a build-up of abnormal proteins in the frontal and temporal areas of the brain, affecting someone\u2019s speech and language.<\/p>\n They are not sure what causes these build ups to appear.<\/p>\n However, previous studies have suggested that it may be because of genetic mutations. This includes changes in the MAPT gene, which is linked to the formation of proteins in the brain, according to the National Institute on Aging<\/a>.<\/p>\n People are more at risk of developing FTD if they are older or have a family history of the disease.\u00a0<\/p>\n The Mayo Clinic<\/a> says there are no other known risk factors.\u00a0<\/p>\n Dr Purcell said: \u2018We are still uncertain of a lot of the causes of FTD in patients.<\/p>\n \u2018It may be down to a genetic component, which is associated with proteins that get deposited in the brain.\u2019<\/p>\n \u2018As a family, we wanted to take this opportunity to thank you all for the outpouring of love and compassion for Bruce over the past ten months.
