High <\/span><span style=\"font-weight: 400;\">blood pressure<\/span><span style=\"font-weight: 400;\"> is defined as the <\/span><span style=\"font-weight: 400;\">systolic<\/span><span style=\"font-weight: 400;\"> number being higher than 130 mmHg or the diastolic number being 80 mmHg or more.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">About 19% of people have <\/span><span style=\"font-weight: 400;\">edema<\/span><span style=\"font-weight: 400;\">, which often causes discomfort, weakness, and a limited range of motion.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Both of these medical conditions often impact a person\u2019s <\/span><span style=\"font-weight: 400;\">quality of life<\/span><span style=\"font-weight: 400;\">. They can also contribute to more serious conditions, such as <\/span><span style=\"font-weight: 400;\">heart disease<\/span><span style=\"font-weight: 400;\"> and other diseases.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Further, the <\/span><span style=\"font-weight: 400;\">management of heart failure<\/span><span style=\"font-weight: 400;\">, in particular, is important because decompensated heart failure can contribute to kidney and liver <\/span><span style=\"font-weight: 400;\">dysfunction<\/span><span style=\"font-weight: 400;\">, as well as the <\/span><span style=\"font-weight: 400;\">worsening<\/span><span style=\"font-weight: 400;\"> of pulmonary <\/span><span style=\"font-weight: 400;\">hypertension<\/span><span style=\"font-weight: 400;\">. <\/span><span style=\"font-weight: 400;\">Heart failure patients<\/span><span style=\"font-weight: 400;\"> are at increased risk for heart conditions of irregular heartbeat, valvular heart disease, and sudden cardiac arrest.<\/span><\/p>\n<h2 id=\"switching-from-torsemide-to-furosemide\"><span style=\"font-weight: 400;\">Switching from <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Treatment is important, but patients may not be satisfied with the medication they take due to ineffectiveness or a change in insurance coverage. For example, some people who take <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> may want to switch to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\"> because of its benefits.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">First, note that <\/span><span style=\"font-weight: 400;\">diuretic therapy<\/span><span style=\"font-weight: 400;\"> is a standard treatment for edema and is often an add-on medication to help control elevated blood pressure. Out of all the <\/span><span style=\"font-weight: 400;\">loop diuretics<\/span><span style=\"font-weight: 400;\">, these two are the most commonly used to treat <\/span><span style=\"font-weight: 400;\">acute decompensated heart failure<\/span><span style=\"font-weight: 400;\">, with the two hallmark symptoms of congestion and <\/span><span style=\"font-weight: 400;\">volume overload<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Further, <\/span><span style=\"font-weight: 400;\">thiazide<\/span><span style=\"font-weight: 400;\"> and similar diuretics aren\u2019t as effective as <\/span><span style=\"font-weight: 400;\">loop diuretics<\/span><span style=\"font-weight: 400;\"> since they can only achieve a maximum <\/span><span style=\"font-weight: 400;\">natriuresis<\/span><span style=\"font-weight: 400;\"> of about 3-5% of the filtered sodium load (aka they can\u2019t excrete as much sodium).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> and <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\"> have benefits compared to similar medications. But can a person switch from <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Clinicians<\/span><span style=\"font-weight: 400;\"> say yes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIt is possible and sometimes advisable to switch from <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">, depending on clinical judgment,\u201d says Dr. Raj Dasgupta, MD, the chief medical adviser for Fortune Recommends who\u2019s quadruple board-certified in <\/span><span style=\"font-weight: 400;\">internal medicine<\/span><span style=\"font-weight: 400;\">, critical care medicine, pulmonary medicine, and sleep medicine.<\/span><\/p>\n<p><a href=\"https:\/\/www.beverlyhillscardiovascular.com\/dr-setareh-bio\/\"><span style=\"font-weight: 400;\">Dr. Sam Setareh, MD<\/span><\/a><span style=\"font-weight: 400;\">, a cardiologist at Cedars-Sinai Medical Center and director of clinical <\/span><span style=\"font-weight: 400;\">cardiology<\/span><span style=\"font-weight: 400;\"> at Beverly Hills <\/span><span style=\"font-weight: 400;\">Cardiovascular<\/span><span style=\"font-weight: 400;\">, agrees. He says both torsemide and furosemide are <\/span><span style=\"font-weight: 400;\">loop diuretics<\/span><span style=\"font-weight: 400;\">, but they do have differences. Hence, there is a question about which is better or if switching medication is worth it.<\/span><\/p>\n<h2 id=\"what-are-the-benefits-of-switching-from-torsemide-to-furosemide\"><span style=\"font-weight: 400;\">What are the benefits of switching from <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">People may want to <\/span><span style=\"font-weight: 400;\">switch from <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\"> for a few reasons:<\/span><\/p>\n<h3 id=\"furosemide-is-easier-to-get-and-take\"><span style=\"font-weight: 400;\">Furosemide<\/span><span style=\"font-weight: 400;\"> is easier to get and take<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Cost is one factor to consider. \u201c<\/span><span style=\"font-weight: 400;\">Furosemide<\/span><span style=\"font-weight: 400;\"> is generally less expensive and more widely available than <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\">,\u201d Dr. Dasgupta says. (Those specifics are in the chart below.)<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Furosemide<\/span><span style=\"font-weight: 400;\"> also comes in more forms (oral, intramuscular, and intravenous) than <\/span><span style=\"font-weight: 400;\">torsemide,<\/span><span style=\"font-weight: 400;\"> which is only available orally. So, if someone doesn\u2019t want to take torsemide by mouth\u2014the only way <\/span><span style=\"font-weight: 400;\">it<\/span><span style=\"font-weight: 400;\"> can be taken\u2014they may want to change to <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3 id=\"furosemide-has-greater-kidney-bioavailability-and-one-dose-works-longer\"><span style=\"font-weight: 400;\">Furosemide has greater kidney <\/span><span style=\"font-weight: 400;\">bioavailability,<\/span><span style=\"font-weight: 400;\"> and one dose works longer<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">Bioavailability<\/span><span style=\"font-weight: 400;\">\u201d refers to a medication\u2019s ability to be absorbed and used by the body. <\/span><span style=\"font-weight: 400;\">Furosemide<\/span><span style=\"font-weight: 400;\"> has <\/span><a href=\"https:\/\/journals.lww.com\/jasn\/abstract\/9900\/mechanistic_differences_between_torsemide_and.409.aspx\"><span style=\"font-weight: 400;\">greater kidney <\/span><span style=\"font-weight: 400;\">bioavailability<\/span><\/a><span style=\"font-weight: 400;\">. It has a longer duration of <\/span><span style=\"font-weight: 400;\">natriuresis<\/span><span style=\"font-weight: 400;\"> (the removal of excess sodium through urination).<\/span><\/p>\n<h3 id=\"furosemide-has-a-different-pharmacokinetic-profile\"><span style=\"font-weight: 400;\">Furosemide has a different <\/span><span style=\"font-weight: 400;\">pharmacokinetic<\/span><span style=\"font-weight: 400;\"> profile<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">\u201cAnother reason could be specific clinical scenarios where <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">\u2019s <\/span><span style=\"font-weight: 400;\">pharmacokinetic<\/span><span style=\"font-weight: 400;\"> profile may be preferred over <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\">,\u201d Dr. Dasgupta says. \u201cThis may happen in cases of renal impairment where <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">\u2019s higher <\/span><span style=\"font-weight: 400;\">bioavailability<\/span><span style=\"font-weight: 400;\"> could offer better diuretic coverage.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, some people may want to <\/span><span style=\"font-weight: 400;\">switch from <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\"> to <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> or simply stay on <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To explain the latter, let\u2019s look at the <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2800428\"><span style=\"font-weight: 400;\">TRANSFORM-HF<\/span> <span style=\"font-weight: 400;\">randomized<\/span> <span style=\"font-weight: 400;\">clinical trial<\/span><\/a><span style=\"font-weight: 400;\">, which assessed the <\/span><span style=\"font-weight: 400;\">effects of torsemide<\/span><span style=\"font-weight: 400;\"> and <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\">. While <\/span><span style=\"font-weight: 400;\">furosemide<\/span><span style=\"font-weight: 400;\"> is the most commonly used <\/span><span style=\"font-weight: 400;\">loop diuretic<\/span><span style=\"font-weight: 400;\"> for <\/span><span style=\"font-weight: 400;\">heart failure<\/span><span style=\"font-weight: 400;\">, <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> may benefit myocardial fibrosis, <\/span><span style=\"font-weight: 400;\">aldosterone<\/span><span style=\"font-weight: 400;\"> production, sympathetic activation, <\/span><span style=\"font-weight: 400;\">ventricular<\/span><span style=\"font-weight: 400;\"> remodeling, and <\/span><span style=\"font-weight: 400;\">natriuretic peptides<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Other <\/span><span style=\"font-weight: 400;\">clinical outcomes<\/span><span style=\"font-weight: 400;\"> from this study were as follows: Patients in the <\/span><span style=\"font-weight: 400;\">torsemide group<\/span><span style=\"font-weight: 400;\"> did not show a significant difference in all-<\/span><span style=\"font-weight: 400;\">cause mortality<\/span><span style=\"font-weight: 400;\"> over 12 months compared to the <\/span><span style=\"font-weight: 400;\">furosemide group<\/span><span style=\"font-weight: 400;\">. The <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.123.064842\"><span style=\"font-weight: 400;\">patient-reported effects were similar<\/span><\/a><span style=\"font-weight: 400;\"> regardless of cardiac <\/span><span style=\"font-weight: 400;\">ejection fraction<\/span><span style=\"font-weight: 400;\">, previous <\/span><span style=\"font-weight: 400;\">loop diuretic<\/span><span style=\"font-weight: 400;\"> use, and <\/span><span style=\"font-weight: 400;\">baseline<\/span><span style=\"font-weight: 400;\"> health status.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So, before making the switch, consider all the <\/span><span style=\"font-weight: 400;\">differences between the two medications<\/span><span 400;\">\u201cIt\u2019s generally not necessary to taper off <\/span><span style=\"font-weight: 400;\">torsemide<\/span><span style=\"font-weight: 400;\"> before starting <\/span><span
