That assumption can lead to mental health stigma for new moms who have complex and nuanced experiences after childbirth. The idea that new parents are supposed to be full of bliss is just one of the reasons that postpartum depression (PPD), also called perinatal depression, often goes undetected.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPostpartum depression can be especially insidious, as new parents expect to feel excited and happy about their babies,\u201d says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/alex-juusela-md-mph-facog\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Alex Juusela<\/span><\/a><span style=\"font-weight: 400;\">, an OB-GYN in Detroit and Maternal-Fetal Medicine Fellow at the National Institutes of Health Perinatology Research Branch at Wayne State University \/ Detroit Medical Center.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Unfortunately, symptoms of postpartum depression go untreated for the same reasons. \u201cPostpartum depression often goes undiagnosed as patients can fear the stigma surrounding depression and hide or not report their symptoms,\u201d Dr. Juusela explains. That\u2019s why understanding PPD and proper screening is so important. There are effective courses of treatment for PPD available, but the cultural taboo around talking about women\u2019s health conditions and mental health renders them unavailable to new mothers with the highest risk factors.\u00a0<\/span><\/p>\n<h2 id=\"what-is-postpartum-depression\"><span style=\"font-weight: 400;\">What is postpartum depression?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Postpartum depression can be defined as depression that manifests within the first year after a person has been pregnant, says <\/span><a href=\"https:\/\/www.veritaspp.com\/team\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Gayle MacBride<\/span><\/a><span style=\"font-weight: 400;\">, Ph.D., a clinical psychologist and cofounder of telehealth provider Veritas Psychology Partners. \u201cPPD is often triggered by significant hormonal fluctuations (estrogen\/progesterone and even thyroid changes), sleep deprivation, physical stress from childbirth, lack of social support, and the challenges of adjusting to parenthood,\u201d MacBride says. People who have experienced depression or anxiety outside the perinatal time are at an increased risk for PPD.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.womenshealth.gov\/mental-health\/mental-health-conditions\/postpartum-depression\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Postpartum depression symptoms<\/span><\/a><span style=\"font-weight: 400;\"> include extreme fatigue, mood swings, irritability, feelings of guilt or inadequacy as a parent, changes in appetite, and difficulty bonding with the baby.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The difference between postpartum depression and major depression or depressive disorder is, as Dr. Juusela explains, really a matter of timing. \u201cThe Diagnostic Statistical Manual (DSM IV) specifies that the postpartum depressive episode begins within four weeks of giving birth,\u201d he says but adds that we know from reporting of behavioral health clinicians that PPD can appear any time in the year after a person gives birth.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, PPD can be confused with other kinds of mental illness, or it can be written off as \u201cbaby blues.\u201d While the \u201cbaby blues\u201d are common and mild feelings of sadness after giving birth, they generally resolve a few weeks after giving birth. PPD and postpartum mood disorders persist for longer and may have more intense symptoms.<\/span><\/p>\n<h2 id=\"how-common-is-postpartum-depression\"><span style=\"font-weight: 400;\">How common is postpartum depression?<\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">One in 8 women in the U.S. report <\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6919a2.htm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">depression in the postpartum<\/span><\/a><span style=\"font-weight: 400;\"> period. (<\/span><i><span style=\"font-weight: 400;\">CDC, 2018<\/span><\/i><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">1 in 5 women experienced symptoms of PPD but did not report symptoms until asked by a healthcare provider. (Centers for Disease Control and Prevention, 2020)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">1 in 8 women are not asked about depression during postpartum visits. (CDC, 2020)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The prevalence of PPD has been increasing in the U.S. <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2811765\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Race is a factor<\/span><\/a><span style=\"font-weight: 400;\">. Of women reporting PPD, 38.6% were white, 6.6% were Black, 26% were Hispanic, and 24.3% were Asian. (JAMA, 2023)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rates of PPD are <\/span><a href=\"https:\/\/www.ajog.org\/article\/S0002-9378%2822%2901132-2\/fulltext\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">higher for obese<\/span><\/a><span style=\"font-weight: 400;\"> and morbidly obese women. (AJOG, 2023)\u00a0<\/span><\/li>\n<\/ul>\n<h3 id=\"postpartum-depression-statistics-worldwide\"><span style=\"font-weight: 400;\">Postpartum depression statistics worldwide<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The World Health Organization predicts that postpartum depression affects <\/span><a href=\"https:\/\/www.who.int\/teams\/mental-health-and-substance-use\/promotion-prevention\/maternal-mental-health\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">between 10 and 20% of the world\u2019s population<\/span><\/a><span style=\"font-weight: 400;\">. (WHO, 2023) According to one study:<\/span><\/p>\n<h3 id=\"postpartum-depression-statistics-in-the-u-s\"><span style=\"font-weight: 400;\">Postpartum depression statistics in the U.S.<\/span><\/h3>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.americashealthrankings.org\/explore\/measures\/postpartum_depression\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Women in Idaho<\/span><\/a><span style=\"font-weight: 400;\"> have the highest prevalence of PPD, at 25.4%. (United Health Foundation, 2023)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.americashealthrankings.org\/explore\/measures\/postpartum_depression\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Vermont<\/span><\/a><span style=\"font-weight: 400;\"> has the lowest incidence of postpartum depression, at 8.7%. (United Health Foundation, 2023)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Between 28% and 70% of <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19161871\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">mothers with infants in Neonatal ICUs<\/span><\/a><span style=\"font-weight: 400;\"> reported symptoms of PPD. (United Health Foundation, 2023)<\/span><\/li>\n<\/ul>\n<h3 id=\"postpartum-depression-statistics-by-race-and-ethnicity\"><span style=\"font-weight: 400;\">Postpartum depression statistics by race and ethnicity<\/span><\/h3>\n<h3 id=\"postpartum-depression-statistics-by-age\"><span style=\"font-weight: 400;\">Postpartum depression statistics by age<\/span><\/h3>\n<h2 id=\"postpartum-depression-and-overall-health\"><span style=\"font-weight: 400;\">Postpartum depression and overall health<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Postpartum depression can impact every aspect of a new mom\u2019s well-being and health. Not only that, but it can also affect the well-being of children and families. \u201cWith more persistent\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">sadness comes loss of interest in activities, changes in sleep patterns, changes in appetite, feelings of hopelessness, and difficulty concentrating,\u201d MacBride explains. Those factors play a role in every facet of physical and mental health. New moms may lose or gain weight in ways that impact their health or engage in unhealthy coping behaviors.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When left untreated, PPD can hinder bonding and healthy attachment to the newborn or distort the mother\u2019s perception of the infant\u2019s behavior.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cBirthing parents may misjudge or over-interpret baby\u2019s signs, or they might miss cues,\u201d MacBride explains. Basically, PPD can impair the birthing parent\u2019s attention and judgment in ways that are detrimental to the new mom\u2019s health and the health of the baby.<\/span><\/p>\n<h2 id=\"causes-of-postpartum-depression\"><span style=\"font-weight: 400;\">Causes of postpartum depression<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">PPD is generally attributed to hormonal fluctuations, but other risk factors have to be taken into consideration. MacBride says that it\u2019s difficult for researchers to pin down exact causes when there are so many variables that go into studies on PPD. But, she says, our culture plays a role. \u201cWe believe that if you are in a culture in which you perceive support after giving birth and can express emotions or worries openly, you probably find culture helpful,\u201d says MacBride. \u201cIf you believe you need to bear it in silence and must bond and be happy about parenthood, you might experience a greater risk for PPD.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Socioeconomic factors, stressful life events, and neglecting maternal mental health also play a role. \u201cNormal pregnancy, childbirth, and the postpartum period is stressful in the first place and can be an emotionally vulnerable time,\u201d Dr. Juusela says. Juusela explains that this already emotionally fraught time can be exacerbated by <\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6919a2.htm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">additional stressors<\/span><\/a><span style=\"font-weight: 400;\">, such as medical complications of pregnancy (gestational diabetes, preterm labor, preeclampsia), infant health, relationship or family issues, intimate partner violence, financial concerns, unstable housing, and myriad other stressors can increase the risk of anxiety and depression.<\/span><\/p>\n<h2 id=\"postpartum-depression-prevention\"><span style=\"font-weight: 400;\">Postpartum depression prevention<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The best way to prevent PPD is to receive proper care and support during pregnancy. Expectant moms who have a history of mood disorders can start or continue taking mental health medications.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cCompared to those who do not receive treatment, pregnant persons who are <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamapsychiatry\/fullarticle\/2526241\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">treated for depression during pregnancy<\/span><\/a><span style=\"font-weight: 400;\"> are at decreased risk of preterm birth, having a baby born with low birth weight, or feeling an inability to bond emotionally with their baby,\u201d Dr. Juusela says. \u201cDuring pregnancy, medications called selective serotonin reuptake inhibitors (SSRIs) are commonly used to treat major depressive disorder. <\/span><span style=\"font-weight: 400;\">SSRI use during pregnancy<\/span><span style=\"font-weight: 400;\"> is effective and generally considered safe.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, if you already have <\/span><span style=\"font-weight: 400;\">risk factors such as a history of depression<\/span><span style=\"font-weight: 400;\">, anxiety disorders, bipolar disorder, family history of mental disorder, mental illness, or postpartum psychosis, make sure you get help when you become pregnant.<\/span><\/p>\n<h2 id=\"treating-postpartum-depression\"><span style=\"font-weight: 400;\">Treating postpartum depression<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are multiple screening tools to help detect PPD, and these are valuable tools. \u201cTwo of the most commonly used for peripartum depression are the <\/span><a href=\"https:\/\/perinatology.com\/calculators\/Edinburgh%20Depression%20Scale.htm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Edinburgh Postnatal Depression Scale (EPDS<\/span><\/a><span style=\"font-weight: 400;\">) and the <\/span><a href=\"https:\/\/www.apa.org\/depression-guideline\/patient-health-questionnaire.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Patient Health Questionnaire-9 (PHQ-9<\/span><\/a><span style=\"font-weight: 400;\">),\u201d Dr. Juusela says. \u201cBoth tools are easy to administer, and both are highly effective at ruling postpartum depression in or out.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">After receiving a diagnosis of postpartum depression, there are many ways to treat it. The following can help alleviate PPD symptoms:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Lifestyle changes: <\/b><span style=\"font-weight: 400;\">First things first, new moms need sleep, healthy food, exercise, and support from family members and health professionals. MacBride says that she sees a lot of new moms trying to muscle through the first few months without enough help and too few hours of sleep. That can be a dangerous combination for developing PPD and other health problems.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Talk therapy: <\/b><span style=\"font-weight: 400;\">New moms need someone to talk to. \u201cTalk therapy can be very helpful,\u201d MacBride says. \u201cMy clients benefit from knowing that what they are going through is normal and it\u2019s a safe place to talk about the things they think they would otherwise be negatively judged for even thinking (although it\u2019s usually very normal and understandable).\u201d If one-on-one talk therapy isn\u2019t available, <\/span><a href=\"https:\/\/www.postpartum.net\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">support groups<\/span><\/a><span style=\"font-weight: 400;\"> can offer some relief. If you need a referral, see your primary care health professional.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Medication: <\/b><span style=\"font-weight: 400;\">In addition to traditional antidepressants, there are also new medications aimed specifically at treating PPD. Zurzuvae (zuranolone) was approved by the U.S. Food and Drug Administration (FDA) in 2023 for the treatment of postpartum depression. It\u2019s an oral tablet taken for 14 days. The other PPD-specific treatment is Zulresso (brexanolone), a 60-hour IV infusion treatment approved in 2019. It does require a hospital stay, so Zurzuvae may be more accessible.\u00a0<\/span><\/li>\n<\/ul>\n<h2 id=\"postpartum-depression-questions-and-answers\"><span style=\"font-weight: 400;\">Postpartum depression questions and answers<\/span><\/h2>\n<h3 id=\"what-percentage-of-mothers-experience-postpartum-depression\"><span style=\"font-weight: 400;\">What percentage of mothers experience postpartum depression?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">One in 8 women in the U.S. report <\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6919a2.htm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">depression in the postpartum<\/span><\/a><span style=\"font-weight: 400;\"> period. The <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41398-021-01663-6\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">global prevalence<\/span><\/a><span
